A special reservation link will be included in your Conference registration confirmation email.Disclosure to LearnersAccreditation Statement: Ohio Council for Home Care & Hospice is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.Relevant Financial Relationships: There are no relevant financial relationships with ineligible companies for those involved with the ability to control the content of this activity\, or any relevant financial relationships disclosed by participants have been mitigated.Criteria for Awarding Contact Hours & Continuing Education Certificates: Attendance for the duration of the educational activity\, missing no more than 10 minutes.Attendee Cancellation PolicyAttendee cancellations must be made by email to ochch@ochch.org at least 2 business days before the beginning of the event. A refund will be issued within 5 business days. If the request is not received in a timely manner\, no refund will be provided. Special considerations may be made in situations of personal hardship/distress. Attendees can choose to swap their registration to another individual at no cost at any point prior to the beginning of the event. In that situation we may not be able to adjust certain pre-existing selections such as breakout choices\, lunch selections\, etc.Exhibitor Cancellation PolicyExhibitor cancellations must be made by email to ochch@ochch.org with the following timing windows. If a refund is due\, it will be issued within 5 business days of notification. OCHCH notified with 14 business days or more until event\, 100% refund.OCHCH notified between 14 and 10 business days prior to event\, 50% refund.OCHCH notified with less than 10 business days to event\, no refund. In this scenario\, an exhibitor can choose to send materials to be placed on their selected space by OCHCH staff\, even if representatives can no longer attend. If the Exhibitor wishes to have any of these materials returned to them\, a pre-paid shipping tag must be created by the Exhibitor and made available to OCHCH staff before the event begins. The original packing method will be used to return any such items.
